Care plans did not always contain sufficient information about people's health needs... care plans could not be relied upon as an accurate record of people's care.

Quality checks to ensure care plans were accurate were not always effective. Audits were completed but had failed to identify gaps in information for staff to refer to.

During inspection we identified 3 people did not have diabetes care plans in place... We also found staff did not have guidance to support people with epilepsy.
